APAAR ID Consent Form
As per the government guidelines under NEP 2020, a consent form is required to create the APAAR ID of the minor students (students aged below 18).
Parents are requested to give their consent by filling out a consent form and submitting it to the respective class teacher of the student, along with a copy of either the mother’s or father’s Aadhaar card.
Once the consent form is submitted the school will verify the details of the student and issue an APAAR ID on the student’s name. After the ID is created you can download it online or your school will provide you the ID.

How to Fill the Consent Form
You can easily fill the consent form by following these steps.
- Obtain the consent form from school or download it from the official website and print it for physical submission.
- Write the full name of the student’s school.
- Mention the Father’s or Mother’s name.
- Provide the name of the student as per Aadhaar card.
- Parents must give their Identity proof such as (Aadhaar / PAN / EPIC / DL / Passport) and also provide an identity proof number.
- Mention the date of form submission in the “Date of Physical consent” option.
- Provide the school’s address in the “Place of Physical Consent” option.
- Carefully review the entered details, and provide the signature.
After filling in the form, attach the copies of the student and one of the parents Aadhaar Card and submit the form to the school.
